US Based Developers vs Foreign

We have been using foreign based developers for our website and mobile app, and for the most part, it works pretty well, but it feels like most tasks take longer than it should to get changes/updates completed, so I'm wondering about US based developer costs, and or companies to check out? Do places charge by the hour, or can you get monthly hours of work?
Keeping our costs down is a big driver as our revenue fluctuates based upon customer activity.
Our product is MySQL with php7 and codeigniter framework on the backend, and HTML, CSS, and jquery on the front end if that helps.
