When I worked tech support, that was typically a bad HDMI cable.
It means the HDMI handshake between your TV and cable equipment failed. Could be 3 things:
1) HDMI port on TV going out
2) HDMI port on box going out
3) HDMI cable going bad or is defective.
I have seen it be a bad TV and bad box before, but definitely switch out that cable first. For every 10 times I saw this, at least 9 times was a bad cable.
