Friend of the family was pretty high up with BCBS until recently. She told us stories of people getting coverage (3 month grace period from buying coverage without payment before BCBS could cancel the policy) and the patient would go get treatment for Hepatitis (perhaps C) which is a prexisting condition. The treatement would cost ~$100k and would finish up about the time the policy was canceled for non payment. BCBS received $0 in premiums from the insured.
She said this was more common than you think because once people discovered this work around, the news spread.

And BCBS was out $0 because they just back billed the provider.
Had it happen to me several times. See patient for 3 months covering multiple visits maybe (4-7) to get their multiple med issues which were uncontrolled for the past 10 years of being doctor less all squared away. Of course do labs, imaging, pay employees, incur overhead all while BCBS verifies benefits each time.
Pt stops coming in after 3 months, at end of the year 6 months later get a bill from BCBS for all payments they made as pt was non insured due to not paying their premiums.
1st year I took the ACO BCBS I had to remit over 7K in payments I had received for work I most certainly performed.
