BOP Failures

Has a BOP ever failed before in the Gulf, North Sea, Persian Gulf, etc.? If so how many times or how frequently? Do some wells have multiple "fail safes"?

I've heard they are 50/50 in working/ failure rate. Just what I heard, no idea if that is truth or not.
