I'm sure there is a lot of both.
I think there is also a lot more "you assaulted/raped me" without actually reporting it to police or anyone of note than there are fake cases. We had a girl who used to hang around our fraternity that claimed someone took advantage of her, and when we offered to immediately call the police and have someone come investigate she dropped it and began apologizing profusely.
Obviously that works out without any true slander of reputation, but I'm sure that happens a lot, and often with some impact (though lesser than actually being charged with a crime or accused in the media).
Anyway, point being that rapes are underreported, false allegations probably go unknown/unreported a lot, and the latter has certainly made problems for real victims.
