That is the classic reasoning... CDC doesn't even have surveillance for rheumatic fever anymore because it so rare in developed nations

True. But if you don't use that criteria to narrow the population you're testing, you're wasting money and ordering lots more tests than need to be ordered.
Though a 50/50 shot, it's very safe to start antibiotics in anyone with a score 4 or higher, and most people would like a very cheap and safe antibiotic to shorten the course of their symptoms. Most algorithms, including the ID guidelines recommend treating with a modified centor 4 or greater.
ETA: sorry. Not ID guidelines. AAFP clinical practice guidelines.
ETA2: ID guidelines suggest it is acceptable to treat without lab testing in adults
