The current situation where not all implanted lenses are covered by Medicare dates back to the mid 1970's at least and discount eyeglasses companies like Lens Crafters lobbied hard at the state and federal level for approval to sell glasses without running up against restrictive medical/pharmacy laws. That effort caused eye things to have a split classification... medical and cosmetic. Any lens or other thing that is judged to offer mostly cosmetic benefit gets little or no insurance coverage.
