Just draw lines through it. Hell, I'd do the same for most of those questions about me; the only ones remotely relevant are address/phone number/DOB.

No, they were quick to make me pay what my ins. wouldn't because I haven't met my deductable. They called the day before and told me it would be around $300 for visit and xray. When I left, they told me I needed to pay $550. I said the day before it was $300. She she I got an injection and that was another $250.
