Subrogation Questionnaire?

We received a subrogation questionnaire from our health insurance (BCBS). My wife had been getting physical therapy for lower back pain. It wasn't caused by an accident or anyone injuring her, her doctor referred her in lieu of taking pain meds. They want us to mark yes or no to determine if someone else is liable but no one is. Does anyone have any experience with this? TIA

United loves to bother me with those damn forms and phone calls. Just mark no if it applies and be done with it.

Thanks. I've never had to fill one out and neither has anyone else in our family. So we were just confused.

No big issue, fill it out and send it in , they just want to make sure no one else may be responsible for paying for it, certain diagnostic codes set it off, if you do not fill it out and send it in . It will delay payment to the provider

Yeah that's what I figured after reading up on our summary plan description.

Chiropractic care and phys therapy always seem to set it off, with United. My crooked spine needs adjusting, and you'd think that my own insurance company could figure that scoliosis wasn't cause by any accident. Annoying as hell to fill out repeated paperwork and field calls for the same condition, year after year.

Yeah my wife actually had been hit by a car when she was 12. And went through care for that way back then. Then she was diagnosed with Lyme last year and that made her have pain all over. Once she was cleared of the Lyme her lower back still hurt and we wanted to start having children so pain medication was out of the question. Her and her doctor decided on the PT, and once she started therapy they actually determined that she has arthritis. She's actually done with the PT now and it seemed to help a lot.
