benefit account tax question

OK guys, my wife opened a benefit account for me last year when I was in a bad accident.
we are trying to figure out what needs to be done to file tax info. She has an EIN number but it wasnt classified as tax exempt.
Anyone have any info on getting this done? What form we need, etc.?

When she got the EIN, what type of entity did she indicate it was?
