I will put an underlayment down. I have ripped up all the carpet and padding and scraped what glue I could, but there’s still some residue left. How important is it for me to get the concrete slab to a smooth finish?

Perfectly smooth is not as important as level. If you are worried though you could rent a concrete sander or put a skim coat on top with this.

I would stick to whatever the manufacturer recommends for an underlayment. If you have a padded underlayment and padded planks, that would weaken the locking mechanism causing you more long term issues.
