How to prevent leaf spot for tomatoes?

Haven't had any real trouble we couldn't solve with any of our other plants, but for the 3rd season now our tomatoes have gotten leaf spot and each time its terminal.
I've changed out dirt, planted in different spots, used fungicide, pruned, etc. Not sure what to do at this point.
Any advice from the board?
