No definitive answers, but according to a couple of Reddit threads, peoples’ working theory is that he dies in his cell if you “enter” Act 2, even for a moment. Did you step into the Grymforge lift prior to getting to him?

Yeah, I read that too and I haven’t been there. Only to the swamp, monastery area, goblin camp and some of the mountain pass. I’m trying to make my way out of the Goblin Camp and went into the Underdark for a bit.

Someone correct me if I'm wrong, but doesn't entering the Mountain Pass count as entering Act 2?

Looks like I misremembered. Mountain Pass should be fine. Maybe an excessive long rest issue? I know those still crop up from time to time and surprise the community.

I went down an 8 hour rabbit hole and picked up Phalar Aluve, Blood of Lathander, and some good gear. I used the portal to get out of the Inquisitor’s area and went back into the main world to smash everyone who made my life a living hell. I’m going to knock out these side quests before I head back.
