You'll be fine at PM and underground with jeans and button up. You'll see people coming straight from work wearing suits. I've seen people eating at underground during shows which I also thought was kinda odd. I've never eaten there so I can't comment on their food but I hear it is good. They do have a pretty decent beer selection.

Yes, they are usually still serving during the performances. If you're there before 7:30, you can avoid the $10 cover. Otherwise, you can go whenever, but it can get crowded quickly with the better acts.

Did Parlor Market and 119 a few weeks back. Great time. I had the duck confit carbonara at PM and it was delicious.
