Retirement request for FAT arse

Guys
A coach I use to coach with in Chicago is retiring after this season. As a joke, his staff and me want to send him out with a bang. We are planning to post a picture of him forever enshrined in the homecoming program. Here is what we had in mind:
Replace the face of Fat David:
With his: (sorry in advance of viewing his mug)
And replace the text to say something on the order of:
"The Living Legend of Libertyville - NAME REMOVED (but see below for example)"
