It's probably worse to "need" ECMO more than a ventilator. However, you don't get the iatrogenic barotrauma with ECMO as you do with high pressure mechanical ventilation. As I recall, they were having problems with patients getting worse with mechanical ventilation (traditional) and were switching to BiPAP and CPAP to maintain oxygenation.
