Not saying the infosec team doesn't hold and responsibility here, but it's impossible to say until more info emerges who's truly to blame (though I bet someone in infosec takes the heat). Time and time again I've seen incidents like this occur after the infosec team had raised the red flag, but the issue was ignored due to the C levels not wanting to let go of the purse strings.

I'm not sure about this. Infosec usually controls stuff like this and tightly too. It's not like Colonial is going to share that info either.
