You assume none of that would have happened without the ACA? Possible, sure, but there has been basically no change in the trend of total underlying cost increases.

It's not the cause, but it is 100% a cause for damn near doubling premiums.

That's the whole joke about "affordable" health care act. It's affordable to those who benefit from it. For the rest of us, our premiums damn near doubled to make up for those who are getting their shite paid for.
