First of all, I agree. It's never okay... until it's a conservative. Then there's no limit.
Now, Trump isn't morbidly obese, but he is certainly overweight. Identifying that is a truism. It's expected people will call him out for that because Trump has a history of calling other people fat. That's their way of trying to get back at him. They'll try anything to get under his skin, and knowing Donald, calling him fat probably does bother the hell out of him.

You think calling trump fat bothers him? Let me tell you something. Trump is rich. Very rich. If he gave a shite about being overweight, he wouldn’t be. It’s not hard to be skinny if you’re rich.

He's not only not morbidly obese, he's not obese at all. Maybe slightly overweight, but for a man his age he's in pretty good shape.
