I've left $0.01 when I have received terrible service. It has to be downright terrible, which is rare, but it has happened.
For your experience I would've left about $10-$15, but that's strictly based on your description. Tipping is just that: something extra for exceptional service. This idea that tipping is required is absurd; it's fundamentally opposed to the definition of "tipping." It's a reward for service, and it should directly reflect the service received.
I am usually a 25% tipper. Most times I feel service is good. When it's poor I like to get an understanding of why. If it's beyond the waitress's power I tip her well. Your case could've been a simple "oh shite I completely forgot" case due to being busy, having a bad day or some other honest happenstance. In that case I would still tip the fair amount of 20%. It truly is situational.
