People who don't deal with contracts put way too much stock in them like they're these invincible hallowed objects that you are allowed to do anything you want to enforce and no one can ever break.

The airline didn't do that. They called airport security, as they should. The doctor was flailing his arms trying to keep security away from him. If they choose, they could lock the doctor up for trespassing, interfering with the operation of a flight....the charges could add up. They will settle for a small amount of money and a couple of free tickets.

As for the lawyer who would love to get this case? You would lose. You haven't even seen the carriage agreement, and you think it would be a slam dunk? I have major reservations that you are even a lawyer, to make such a statement.
