When was "not really insurance" ever explained prior to passage?

If he got hit by a truck or had a heart attack or developed cancer, the insurer would have paid for his treatment. That's exactly what insurance is.

"not really insurance" is exactly what's available on the exchanges. A "plan" with $12,000 deductable for someone making $24,000 a year isn't insurance. It's a waste of money. They will bankrupt either way. They will receive reimbursed indigent care either way.
