On the rare occasion there is a worm or two in a fish, i generally either cut it or pick it out. If there's a ton, I might throw out the filets.
Duck breast parasites (that cause 'rice breast'; the rice looking stuff is not actually the parasites if I remember correctly but they are caused by them) cannot live in humans but given how squeamish my wife is already, I just toss them. It's been pretty rare in my experience so not a huge deal.
