Ppl need to stop guessing 'who' killed him. members only jacket represented everyone he screwed over over the years. could have been anyone.
see tony nearly dying in episode 'members only.'
I've seen people say he lives and the makeup of the scene is meant to represent he's always living having to look over his shoulder. the only problem being nothing he does in the scene indicates he's paranoid or looking over his shoulder.

I originally interpreted the scene as ambiguous - maybe he lives, maybe he dies, but if he lives, yeah- he's going to have to look over his shoulder.

I'm not so sure. Certainly from a framing, lighting, shooting and the directorial touches - that bell ringing and the chaos of the diner certainly invests the scene with a paranoid vibe - maybe I'm just a paranoid guy. I agree with you that Tony doesn't seem overly paranoid, at least outwardly, in the scene, but he is in NYC (not Jersey, where he would feel safer), there is a lot of activity, and the camera does lurch around as if to suggest Tony is looking for threats, assessing possible threats, etc.
So, it may just be a difference of interpretation.
Ultimately, I think the last scene - the fade to black - is Tony's last moment. Chase's statements are all over the place on that final scene, which is why the ambiguous ending folks (of which I was one) will continue to prevail.
