How the hell do you run out of fuel in a plane

When they did the flight planning they loaded what should have been enough fuel for the length of the trip with adequate reserve based on the normal fuel consumption per hour for the plane. However, there was a problem with the engines running rich and consuming more fuel than normal, that was not factored into their calculation and they ran the tanks dry. Whether they had working fuel level gauges or not I don't know, but they might have ignored them anyway, thinking they were inaccurate, something not uncommon for planes that old.
