Who to blame: Strength/conditioning/medical staff or front office/headcoach

At the end of the day, Loomis and CSP pulls the trigger on draft selections as well as free agents. In between the visit with a team and us taking a player, as far as we know, the medical staff and the strength and conditioning coaches evaluate a player.
In 2013 -- we were healthy until Greer's knee decided to give out and Vaccaro broke his ankle, our team played VERY well up until those two points. Jahri Evans back started giving out, Marques Colston started looking old and the rest is history.
In 2014 -- we absolutely SUCKED on defense, even when healthy. Our team couldn't generate a pass rush, but then Byrd went to IR, injuries to other safeties piled up, Lewis' knee decided to become a balloon, and we couldn't keep a consistent group of DBs. Evans and Colston looked that much older as well.
In 2015 -- we started seeing the issues with health once again, this time Byrd continues to rehab slowly, new FA acquisition Spiller was signed and reportedly had to get surgery before the season started, newly traded LB Ellerbe (who had an injury history in Miami) was gone for 10 out of the 16 games, and once again Lewis (hip), Colston (who knows what), Evans (back), etc were nursing injuries or their health declined even faster. Missing games, missing time. We took on players such as Kikaha, who had a history of knee problems (although he was clear for over a year of any knee troubles) and didn't evaluate Swann very well for his concussion problems until it was too late.
In 2016 -- we continue off this season with Kikaha destroying his knee, prized 1st round pick Rankins having a freak injury, Lewis' rehab going poorly and from the looks of it maybe career ending, Spiller's questionable health status, Ellerbe's quad acting up again to the point he's missed the first 3 games, Breaux being injured in game 1, Armstead nursing an injury in the offseason which seems like carried over into the season already, and a plethora of other injuries that seem to be crazy.
TL;DR, all these injury and problems. Who's at fault? The guys clearing medical status (Ellerbe, Kikaha, Lewis, Spiller, Swann) or the front office/head coach for taking them anyway

Who knows. Lewis didn't have medical issues upon coming here, nor did Swann. I think Swann has just been unfortunate. We shouldn't have let Lewis play hurt, and that is on both the medical staff and the coaches.
Whoever approved Spiller and Byrd in free agency when they were still very injured is the worst.
