I guess they can come loose and fall off.
SO had a flat. The offending object was the pin from a receiver hitch. Don't know where the hitch wound up but that blunt arse pin sure ruined a tire.

I'd rather have the hitch in the receiver than somewhere else in my truck if I was in a wreck. That thing could fly around in the cab of your truck and hurt you pretty badly.
