I posted this same thought a while back after a similar tragedy...
Basically our morning commutes are on autopilot. Same route, same routines, day after day.
I understand how this can happen and it's one of the saddest mistakes anyone can ever make. The parents who do this don't want to kill their children. They made a mistake and this mistake is forever.
I was paranoid about this when my son was a baby. My wife and I would call each other every morning as soon as we got into work to make sure every thing was ok.
Never had a close call but again, I can see how it occasionally happens. We just have too much shite going on in our lives and in our cars. Also, rear-facing car seats, while safer in accidents, do increase the likelihood that a distracted parent might overlook their child still in the car.

Last week, someone posted on FB a "cute" story about how she was taking her 2 y.o to daycare on her way to work and on habit, turned down the road which would take her straight to work. Her daughter spoke up and said, "Mommy, you're going the wrong way!"
Most of the comments were LOLs or people saying how smart the kid was to notice, but all I could think about was the stories like this, and what could have happened if the kid was asleep and didn't make the mom realize before it was too late. Yeah, it's likely that Mom would've noticed anyway, but who knows?
