I don't know, Tank and Morpheus seem to be highly specific to the exact moves Neo needs to make when he is in the Matrix. Highly accurate down to the very second. They know when agents will be where and how Neo can avoid detection by them. I think they can see exactly what these people are doing in the Matrix.

I agree with you, but my point was that's because they are concentrating on Neo, I don't think they can see every person hardwired in all at the same time, and they definitely can switch between different scenes, so it's possible that Cypher just flew under the radar one day and had dinner with Smith.
No one expected his betrayal, no one was looking for it, so if he went to the Matrix for whatever reason, there's no reason for Tank to just stare at him the entire time.
Plus, they all have phones, so Tank doesn't switch to their screen until he gets the call from them, you can see him switching screens in the movie everytime he gets a call.
