Only Ned and Howland Reed were the survivors of the tower of joy that could have known. Obviously neither would have said anything.

Because I think Tywin should have realized it's highly likely that Lyanna died in child birth, and that 3 members of the Kingsguard, one of which is Rhaegar's best friend and the other being the Lord Commander, were there indicates they were guarding the actual king. It's clear from the Tower of Joy sequence in the book that they knew what they'd find when they reached the tower.
