According to the San Francisco Brewers Guild there are 20 breweries in the city--some of which I haven't tried yet.
I would place the following in the top tier of SF Breweries:
Cellarmaker
Almanac
Magnolia
21A
Anchor
Beyond that its a mixed bag and ones I haven't tried. The closest to me (Social Brewery) is average and since its only 5 blocks away, I really should go more often.
The one I haven't gone to yet that I'm most interested in is Barrel Head Brewhouse

Ranking the breweries in Portland is tough. I guess it just depends on what you want. I'll give a rough top 10.
Different styles makes for apples to oranges comparisons.
Cascade - great sours. Clearly the best in that category.
Hair of the Dog - Some fantastic barrel aged beers. Again, gets points because of the uniqueness of their beers.
Deschutes - Best brewpub. So busy, nobody goes there anymore. Very good standard lineup, but one-offs and seasonals are great.
Breakside - Very good beers, and always coming up with interesting one-offs
Laurelwood - Like Breakside, but there IPAs run more to the piney side.
The Commons - On the list because they brew a lot of non-standard beers that are very tasty. They brew more farmhouse style beers.
Upright - Similar to The Commons, Belgian beers, very tasty.
Hopworks Urban Brewery - Everything they brew is really good. Their Brewpubs are very cool, also.
McMenamins - Great place to hang out. They restore old buildings and make brew pubs. Consistency is a problem, as they brew on site at all of their brewpubs, of which there are many.
Fat Heads - New to the scene here in Portland. They have a lot of guest taps in addition to their regular lineup.
