This. And sometimes we just have to get the job done on a "hot tooth" because antibiotics and pain meds just aren't going to get an extremely hyperemic pulp settled down. Sucks for the patient, and I sure as hell don't like it on my end. But not everything is a bed of roses.

I rarely did root canals on teeth that were hurting badly. I'd put them on antibiotics and good pain meds, it would settle down in a day or two, then I'd get them in when I could work when I had adquate time available. Typically that was a week later, but no one complained because they would virtually always calm down with antibiotics (even without swelling). If it didn't calm down in a day or two, I'd have them call me and I'd send them to an endodontist. Doing a root canal on a hot tooth that won't calm down is BAD for word of mouth and referrals.
