I have a whole house gas tankless water heater, no other hot water faucet has this problem so I think it's specific to this bath/shower... Especially considering that it doesn't do it when using the bath faucet, just when switching to shower... What could cause this ? in-wall part of the faucet broken?

someone flushing toilets while you are showering or something like that?...

Is it hot with patches of cold or hot with patches of lava hot water?

If it's not the water heater and doesn't happen anyway else it is probably the shower cartridge. Needs to be cleaned or replaced.

Probably the valve going to the shower. (If it's an isolated problem)

the shower cartridge
if so, then wouldn't it do it to both the bath and shower ?

Maybe a leak in the hot water line?

Call your water company and ask for a back flow preventer. It should be free.

Its the transpondster beneath the head flow device. Remove the wall behind the shower and you'll see it pretty clearly. Easy fix with a gorilla wrench.
