Who ignored the security requests after the previous attack, that blew a hole in the outer perimeter that was so big that an 18 wheeler could have driven through it..Was that a Defense, or State dept request?

Hillary had no direct involvement in decisionmaking that saved the two injured Americans.
The CIA agents did not if fact "ignore orders" to go from the Annex to the diplomatic mission. They were delayed about 10 minutes due to an insistence that they leave the CIA compound with an agreed-upon plan of action. Failure to do so may well have resulted in the team not even getting to the compound itself. No American lives were lost nor injuries sustained a result of the 10 minute delay (Smith and Stevens were already dead). The outstanding hero of that CIA rescue was Tyrone Woods, who battled the attackers at both sites ,on a prolonged basis, and gave his life doing so. The other three Americans who died were quick casualties.
