I would say the final fight against Ocelot in MGS4.
-No weapons
-No codec calls
-No equipment
-You basically have to beat him to death with your bare hands
-The music changes over the course of the battle in chronological order of the games' releases and each time the music changes, so does Ocelot's fighting style.
-The knowledge that Ocelot is The Boss' son
The most sickening part of that fight is after you defeat her and the game makes YOU pull the trigger to kill her and advance the story.

That was a cool fight considering the way it closed out the main story of him and Solid Snake. Cycling through the music of each game was a nice touch too.

I forgot about that. That's an important design decision, forcing the player to end her life.
