Also, is the two-footed challenge always a straight red? Is that defined in the Laws of the Game, or is it just an unwritten rule among referees? Are red card offenses fully dependent on the referee?
Anyone know? I couldn't find it in the Laws PDF on my computer, but it was through Adobe Reader's unreliable search function.

I don't believe it's in FIFAs rules, but in the EPL, the referees are instructed to show straight red for 2 footed challenges. I'm not sure how it is handled in other european leagues.

Dependent on the ref/league. The written rules of the game are unsurprisingly broad on purpose in order to lend subjectivity to the decision.
