I didn't think about Rhaegar keeping Lyanna away from Aerys, that's a good point and it kills the second part of my theory.

A quick thought about the antler and the Direwolf-
another possibility is a foreshadowing of Baratheon's actions (or in actions re: the Lannister power play in KL) leading to the near annihilation of the Stark line altogether.
This was my first thought on the re-read.
ETA: Not to say I don't agree completely with the other interps above
A thought on Ghost:
The pup born of the same litter, but different completely than the others. Shares their blood, but not as closely as they do with each other (the red coloring of the others: Tully blood, of which Jon has none)
Ghost is white coloring...white fur...white hair...Targaryen hair? White-blond, anyway...
and the opened-eyes...an allusion to seeing...and truth. Truth of his parentage, of his destiny?
