That Simpson gave?

It was likely a "me and you" sign to Cowan.

There was a runner on first at that time. So Simpson was letting Cowan know if there was a comebacker to the mound and play at second, he would be going to Simpson (who would be covering second) and not Milam.

That way, if Cowan was put in that spot and turning to second, he would know who the ball was going to. Instead of turning and just throwing directly to the bag.

Various players do it throughout the game, outfielders as well.
