You pay that fee. Whether you realize it or not, or whether it is outright stated or not, you pay it. Any business that does card transactions is not footing the bill for a per-transaction charge. I've actually been in three different restaurants in the past few months that have signage that openly states there are two prices, a cash price and a card price, with the cash price being cheaper.
You do pay a fee.
