If gender isn't about genitalia, but is about some sort of "society roles", why is surgery that mutilates the genitals considered "gender-affirming"?

Exactly and why is there a need to always make a physical/appearance change. Regardless if someone views gender as the same or different than sex, it is undisputed that you cannot change your sex. You can get your genitals mutilated, but you can never become the opposite sex.
