Not being hostile, but a pharmacy benefits manager can nix coverage for anything you are on. I’ve been on a medication covered by this insurance company for two years. It turned my health around for the better. The pharmacy benefits manager just denied it, asking for a pre-authorization from the physician and then turned that down. $600 out of pocket per month, which pales in comparison to some.
