That was my first thought too. Those can look like scenes from horror movies and can kill you in minutes. He probably swallowed blood, vomited blood, and then aspirated blood uncontrollably. That would have been catastrophic in a cramped airplane with no resources.
Poor guy. I feel terrible for the family that had to watch that play out with absolutely no way to stop it :(

I’ve taken care of only a handful of patients with esophageal varicies in my years in the ER.
Bloodiest mess I’ve ever seen. EVER. and I think only one of them survived and they all went directly to the OR stat.
I cannot even imagine what went through a lay person’s mind seeing it
