Hospitals and insurance companies come to an agreement of what's paid (under contract), no matter what the hospital charges on paper. A hospital may charge $900 for a service, but the insurance may cover a portion, $280 for example. And there are laws stating if you're in-network, the hospital can't go back on the patient for the remainder of that charge. I went through a lot of this recently, and the insurance company explained all of this to me.
