My theory: Instead of a clinical psychological problem, this phenomena at its extreme is instead spiritual demon possession.
People who purposely ugly-fy themselves OR trans-ify themselves are at their core, self-loathing and especially anti-God; literally possessed by the rebel Baphomet spirit -- demonic. (The Baphomet is a mockery of God by being a combination of both genders -- with the head/face of a beast -- a Goat).
In the inverted, upside world of Satan, Ugly and Evil = Beauty and Good. And of course Man becomes a Woman and Woman a Man (A Baphomet/ Transformer). Demons are disembodied Spirits -- having no body of their own -- also take great delight in spitefully "helping" God's own creation corrupt herself/himself spiritual as well as physically).
That kind of self-loathing gay women believes that by spitefully destroying and vandalizing their own (outward) feminine beauty, they are "dissin'" and mocking The Almighty --"defacing" God's creation OF beauty and His "purpose" for them. (Purposely "ugly-fied/trans-ified "Men" are doing the same -- mocking God by spitefully ruining themselves.)
Damn, this is definitely the kind of crazy I come here for just for the entertainment value.

Glad to oblige
Question for "Crazy / Entertainment" bookies at the OT:
What's the Under / Over (in inches depth) for subject analysis?
