Never. Do not have anything to do with them, one way or another. They will draw you down into a vortex. Anyone who has had experience with them would tell you the same. There is no "right side" with them. Just a world of pain.
Smart advice... like I said, frick an HR

Then straight to your boss or her boss. I'd probably go to her boss first and then let your boss know about the situation.

Tell the person that told you she is high to report it to her boss

But when is safety not an issue. If you are in a plant, it can be concern for sure. But safety is no less an issue just because it's in an office... Are there stairs, bathrooms, etc.
Sorry, but take it to you boss and hopes he does what he needs to do.

Tell your boss and pass the responsibility on to someone else

This. Tell your boss that one of your employees noticed "X" acting strangely, and then you noticed it too. Let them deal with it. Do NOT go to HR. Preferably, ever.

It needs to be addressed and documented. One way or the other. So much depends on information you have not provided.
Small local business or large corp? What is your role? Authority?
The subordinate obviously is under you, but is the coworker?
Sorry I am not much help, but do this - CYA. Once someone below you alerts you to some information, you are now put to task.
At the very least a verbal warning with witness and documentation , but again - depends on your company and position.

Report to her boss. It doesn't matter if it's a safety concern. It could still be a liability concern. Plus, who the frick wants to work in an environment like that. I like to party as much as the next guy, but work ain't the place for that shite.
This...her boss is responsible for following through to determine what is going on, and taking the appropriate action. Your employee did the right thing taking it to you...you are now obligated to act on that information.

Fire you and hire someone that knows what they are doing.

Evil Little Thing needs to report in. She's one of those HR douches.

Go up to her and yell "are you high?" loud enough for your boss to hear
