re: Why was the Dreamcast not successful?

It failed for a lot of reasons:
- Sega had lost consumer confidence with the Sega CD, 32X, CDX, and Saturn. Also, rumors of yet another system during the Saturn's life span, the Sega Neptune, didn't help matters.
Dont forget the retailers and game publishers. A lot of them were royally pissed at SEGA for all the hardware they released and abandoned in such a short time. SEGA doubly screwed up by following the CD and 32X discontinuations by releasing the Saturn months ahead of when they were all told it was supposed to come out.
Many publishers and retailers refused to work with SEGA after that. For example, EA stopped publishing games for them and EB Games stopped selling their stuff over it. (this discontent also made it easy for game retailers to accept Sony's payments to move their stuff to the front of stores while moving Nintendo and SEGA to the back)
What's even sadder is a lot of the crap that went down with the fast discontinuations and early release stuff was purely due to SEGA of Japan and SEGA of America having a pissing contest with each other. More than anything, SEGA was brought down by the company having divisions that refused to cooperate.

I mostly agree with all your points, especially your comment about the in-fighting of Sega of America and Sega of Japan. However...
Additionally, the launch of the Dreamcast was the most successful single day of sales Electronics Boutique ever had in their history at the time. LINK
We did, however, definitely push the PS2 more than other systems, even after the launch of the Xbox and Gamecube. I remember the manager of our store was told by corporate to gut 5 or 6 new PS2 boxes, and stack them together with tape to make a display to put outside our store to let people know we had PS2's in stock. I always felt bad for those customers who'd end up buying one of those gutted PS2's that needed the box from the display. The manager would have to rip the tape off the box and it'd be all beat up looking.
"Here's your brand new PS2 in a box that someone tripped over and spilled Orange Julius on. Enjoy!"

I doubt this because I bought my Dreamcast from Electronics Boutique. I also sold Dreamcast hardware and software while working there myself.
EB Games stopped selling their stuff over it.

I have like 16 consoles now from Atari 2600 to PS3 and Wii U
PS2 is the only one that's ever gone out on me and had to be replaced (never owned any Xbox)

Ahh, I may have gotten the store wrong then. I know there was at least one major retailer that cut bait with them. Maybe it was WalMart? I'm too lazy to use the google machine.

Because they no longer exist. I think they shut down about 5 years ago.
