Microsoft Outlook Help?

For Custom forms, I go to:

- New Items
- More items
- choose form
- choose from the drop down where I want the form to go

Am I correct?

Also, I made some other folders / options that appear in that drop down menu - how do I delete and get rid of them?

Any help appreciated. This is driving me insane!
