Honestly I my wouldn't surprise me if it was some medical condition like an autoimmune disease or something to that nature. I agree he looked a little down and those types of conditions you typically keep to yourself
If it was something like a knee or ankle then why keep it a secret. Normally musculoskeletal injuries are told to the media. Also we never heard about him getting injured.
Just my speculation.
