A DB doing a form tackle correctly would probably result in a personal foul these days since the helmet will be right under the receivers chin and cause the requisite "looks bad" to get a flag, unfortunately.

That's only for a straight up tackle. And that's going to be on a runner where the tackler can lead with the helmet all he wants. The helmet rule is only for QBs in the pocket or defensless receivers.
You are right about the "looks bad" calls though.
Most tackles are at an angle where the proper form is the defender putting his helmet between the runner and where the runner is going, then he wraps, squeezes and drives.
That type of tackle is almost lost in the NFL.
