The cause of urticaria is rarely identified. Antihistamines, steroids, immunosuppressants, xolair(in chronic urticaria) are the treatment of choice. Allegra 180mg every morning and Zyrtec 10mg every night without skipping a dose of either. Do this religiously. An IM steroid injection may give her some relief as well. I'll do this for 3-4 was and if still having outbreaks then add doxepin. Doxepin is a trycilic antidepressant that has very strong antihistamine properties. If this fails we consider methotrexate, I haven't had very good luck with this drug and urticaria unfortunately. Xolair has showed promise in treating chronic urticaria but it is very expensive.
